Summary

The course is designed to provide learners with the knowledge and skills required to prepare and implement activities within the care plan format and approved care pathways.  Staff will learn how to create, implement, react and review those activities in a person centred way and to achieve the level of knowledge required to be confident in client risk assessment.  The course includes time for the tutor to support the manager/s to discuss and review the current care plan format if adaptations are needed.

The end point goal of this training is to achieve an effective person centred care plan format that managers are confident in using and that they can subsequently cascade to front line staff  to improve their interventions  within the activities of daily living for people they support.

Who should attend?

Area Managers/ Managers and supervisors who are involved in client assessment and in maintaining care plan models for service users.

How many candidates may attend?
This training is not limited in attending staff.

Equivalent Relevant information

1. Certificates are issued for each learner and if dated, are valid to that period. N.B Learners attending must complete the entire duration of the training to qualify for a certificate.

2. A learner assessment may be required post training for audit and PDP purposes.
